Puzzle Box of Yogg-Saron is a rare archaeology artifact that requires 375 skill to find and 140 Nerubian Archaeology Fragments to solve. Up to three Nerubian Obelisk keystones may be used to complete it.

According to Muradin Bronzebeard, the Puzzle Box of Yogg-Saron was found near the fallen necropolis Kolramas in Zul'Drak, and he never heard of it being opened, hoping it stays that way.

• While several quotes are references to the works of H.P. Lovecraft, certain quotes originate from other games, including Planescape: Torment and the Baldur's Gate series.
• Several of the quotes seem to refer to the Old God N'Zoth. Others could have been foreshadowing for Y'Shaarj.
• The item as a whole is probably inspired by the Lament Configuration from the Hellraiser movies.
• The many-eyed goat is mentioned again in Wriggling Mass (quest).
• The Puzzle Box appears as HS Puzzle Box of Yogg-Saron (an epic mage spell) in the HS Saviors of Uldum expansion for Hearthstone. When used, it casts 10 random spells at random targets, mimicking the effect of HS Yogg-Saron, Hope's End. Its flavor text reads: "PUT THE BOX DOWN, RENO!"
