When the title was discovered in the game files, there was much speculation about whether it would actually be awarded, and if so, what it would be a reward for. Some speculated that it would be the next version of the Patient, awarded for using the Dungeon Finder. Nefarian's quote after Maloriak's death pointed to the title being related to Blackwing Descent, but it was not until December 20, 2010, when Premonition killed Maloriak on heroic mode on a live server, that the title was confirmed.
