The Key to the Focusing Iris drops from Sapphiron in the 10-man version of Naxxramas. Although it is a quest item, it must be rolled for, meaning only one person in the raid will be able to loot it, receive the quest The Key to the Focusing Iris, and receive an item Level 213 quest reward item for completing the quest Judgment at the Eye of Eternity, which involves getting a raid to kill Malygos.
