The Tormentor's Notes are found on the corpse of Subject 638, found at near the Stygian Incinerator at Zovaal's Cauldron in the Maw. As Maw lore, Ve'nari will accept it for reputation.

In the beta, this item could be read:

• Deciphering Death's Intentions

• In the beta, its flavor text was: "A crumpled and singed page describing preferred methods of interrogation and subjugation. Ve'nari may find it of interest."
• During the alpha, the item was renamed from Experiment 72 Notes: 638MW-F-K. Back then, it had the following flavor text: A crumpled and partially burned page describing an experimental torture procedure. Ve'nari will be interested in this.
