Talisman of Binding Shard

Talisman of Binding Shard is a legendary item, notable for only ever dropping once. It was looted from Baron Geddon in Molten Core by Noktyn-Archimonde US of the guild Nurfed on March 23, 2005. The item is no longer obtainable; Blizzard removed it from the loot tables the same day Noktyn obtained it, but allowed Noktyn to keep theirs.

According to Travis Day, Talisman of Binding Shard was used by Blizzard in a test version of the Thunderfury quest line. The quest was removed and was not publicly released, but Blizzard forgot to remove the item from the loot table.
